House Rule Idea: HP Recovery Rate & Lasting Injuries
Okay, here's an idea for a D&D HP hack.

If you're not dead when a fight's over, then all of the damage sustained goes from Lethal to Nonlethal and you recover normally from there. (In 3.X, this speeds up natural healing significantly; you from Level/Day to Level/Hour, which can be further improved with Heal checks and stuff.)

Related Idea:

Every time you sustain a Critical Hit, have to make a Massive Damage/Instant Death save or go below 0 HP your PC gets an injury that has mechanical consequences. (See 3.0 or 3.5 DMG for examples.) These may be graded as Minor, Light, Serious or Critical and requires a Cure Wounds spell of that grade or better to magically fix.

I like the nonlethal damage idea. Permanent criticals can be fun for one injury that has only minor consequences. More than that, people will want to roll up a new character.

Maybe a Fort Save to avoid the injury, w/ DC varying by grade?

I toyed around with this sort of thing (in my case it was called shots) and the trick is that it makes certain builds/strategies (in this case, increasing crit range) way more valuable. Also, making it depend on crits hurts spellcasters in comparison.

Whether or not this is a problem for you of course depends on how well balanced you think the game is by default and how much you care about balance in the first place.

If you expand it to include Massive Damage checks or when you go below 0 HP, then raw damage output also gets its day in the sun.
